As you can see, we already have the plugin installed. To install Forminator, click “Install”.

Once Forminator is installed, click “Activate”, and you’re all set up.

Step #2: Open Forminator and Select Your Form Type

Once Forminator has been installed and activated, you should see it on your WordPress dashboard:

Next, click on the plugin and you’ll be directed to the dashboard. From there, click “Create” under the “Forms” section:

Finally, you’ll be able to choose from various form types. In this example, we’re going to select “Quote Request”:

Step #3: Adjust the Style of Your Form

Now, here comes the fun part.

With Forminator, you can create a dynamic form in just a few minutes.

Take a look at the settings of the form we just created:

As you can see, we only used five fields:

  1. Services
  2. Name
  3. Email Address
  4. Phone Number
  5. Service Description (What do you need?)

The idea is to create a field that will show up on your form. Forminator offers several field options like:

You can choose whichever field you want, but let’s only use our five.

We used the “Checkbox” field to allow users to select which service they wanted from us:

As you can see, we listed three different service options to choose from:

  • Content Strategy
  • Content Writing
  • Link-Building

Finally, we labeled the form as “Services” to show up like this:

The next steps are pretty simple. We added name, email address, and phone number fields like this:

Finally, we added a “Text Area” field to allow users to explain more about the service they want:

Then, we simply labeled the field “What do you need?”:

And, that completes our Quote Request form.

Step #4: Preview and Publish Your Form

Once you’re satisfied with your form, you can preview it to make any last changes. After that, you’re free to publish it:

Once you publish your form, you’ll receive a shortcode you can paste on any web page, and it’ll show up:

And, voila!

With Forminator, you can easily create a Quote Request form and begin capturing leads for your business.
